Continuity in Family Medicine Elective
The rural “LIC” was designed in 2016 and piloted in academic year 2016-17 by Autumn Haynes D.O., an OMS III student at the time. Designed by Randy Longenecker, M.D., and further refined, and now directed by Scott Rogers, M.D., and Sharon Casapulla, Ed.D., MPH, it has been expanded to include urban underserved settings.
Continuity is a basic property of primary care, and LICs are their parallel in medical education. Students benefit from longitudinal relationships with preceptors, patients and place, and are much more likely to choose a career in primary care.
This elective is an introduction into real medical practice, allowing students to develop continuity with a preceptor and a panel of patients over one academic year. Students in the LIC cohort are placed with a rural or urban family medicine practice to ensure they are exposed to the continuity of primary care beyond the typical month long rotational model in medical education.
Students spend one full month at the beginning of the third year of medical school completing the required Primary Care rotation at their CFM-LIC practice. Students end the year-long LIC course with a month-long required Family Medicine rotation at their CFM-LIC practice. In between the two month-long rotations the student will spend a half day each week or one full day every other week at their CFM-LIC practice for the entirety of the academic year.
During their LIC experience students develop their own "mini practice." They gradually acquire a small cohort of patients they follow over the age, gender, and diagnosis spectrum of family medicine. Throughout the year, students have the opportunity to identify, conduct and implement a quality improvement activity in the practice site or complete a community engagement project.
Overall, 59 HCOM students have completed the CFM-LIC since 2016
· 17 HCOM students enrolled in the CFM-LIC for 2023-24
· 15 HCOM students are enrolled in the CFM-LIC for 2024-25
· 10 of 14 (71%) CFM LIC participants in the class of 2024 matched into a Primary Care specialty
· 8 of 14 (57%) CFM LIC participants in the class of 2024 matched into Family Medicine
